Our Commitment

Our commitment is to provide a nurturing home environment where adults with developmental and complex needs can live safely, grow in independence, and receive the support they need to lead fulfilling lives. Through thoughtful care and a respectful approach, we aim to make a positive and lasting difference in the lives of those we serve.

Role in the Community

Rephidim Supportive Living programs are developed using a client-centred approach, meeting clients where they are, rather than expecting them to follow an eligibility intake process. The program is evidence-based, meaning that we document each adult’s needs using our Service Overview document, which is reviewed and assessed every 90 days. Additionally, conducts monthly clinical Service reviews to ensure adults are promptly connected to community services and support.

We are committed to creating environments where every resident is supported to live with dignity, independence, and a true sense of belonging. Our homes are designed to offer more than just accommodations; they provide comfort, consistency, and a sense of security that allows individuals to thrive.

We understand that adults with developmental and complex needs may require varying levels of support in their daily lives, and we take a person-centred approach to meeting those needs with care and respect. Our focus is on fostering independence while offering the right balance of guidance, encouragement, and practical support. We work to ensure that each resident feels valued, understood, and empowered to participate meaningfully in their home and community.

Rephidim Supportive Living is committed to learning, adapting, and fostering diversity in our programs, homes, and staff. As such, is dedicated to fostering working relationships with various community agencies that can provide service and support to cater to the needs of the adult in our care. Additionally, strives to consider that staff reflects and represents similar or the same culture, ethnicity and traditions as adult to support having adequate representation of themselves.

Our Focus

Rephidim integrates a set of core features designed to support safety, stability, and growth for adults in our programs.

  • Immediate placement

    • Crisis-responsive admission options to ensure adults in urgent situations receive rapid access to safe and stable care.

  • 24/7 supervision (where required)

    • Round-the-clock staffing and oversight in homes where continuous support is necessary to protect well-being and manage complex needs.

  • Basic needs and daily living

    • Consistent support with housing, nutrition, daily routines, and personal care, ensuring fundamental needs are met.

  • Support services

    • Access to counselling, education support, mental health resources, and life skills development, addressing the broader social, emotional, and practical needs of each adult.

  • Dedicated medical follow-up

    • Partnership with a nurse practitioner who provides ongoing medical follow-up, including mandatory medical visits delivered in person or virtually, addressing a common shortfall after foster care ends.

  • Transition planning and collaboration

    • Close collaboration with placing agencies, social workers, and healthcare providers to create and implement individualized transition plans, including long-term placement options and support strategies.
